List exposures
Every way into the app — the hostnames it answers on and the ports published on its node.
verification is derived: not_required for a port or a hostname under our own apex, then pending, failed or verified for a domain the customer brought. Only a domain that is not pending or failed is actually routed.
isManaged marks an exposure the platform created with the app. Those cannot be deleted or repointed — deleting one would strand the app — but they can still be disabled.
